Informed Consent to Nutritional Therapy and Kinesiology with Lily Lee (Dip CNM, ICAK), T/A Innate Health.
Nutritional Therapy is practiced by non-medical professionals and is complimentary to other regulated forms of healthcare in the United Kingdom. Consultations with a nutritional therapist include taking a detailed case history, performing a physical exam if appropriate, creating and providing a personalised nutritional plan, lifestyle advice, supplement recommendations, functional testing, and following up on results and progression of your health. Nutritional therapy employs a range of therapeutic techniques including dietary and lifestyle advice, supplement recommendations and functional testing. While the best course of action is continually sought for the patient there always exists the possibility of side effects, adverse reactions or inefficacy of treatment. Innate Health holds your safety and well-being as their top priority in the management of your case and welcomes all questions or concerns you may have.
Applied Kinesiology is practiced by suitably qualified health professionals and is complimentary to other regulated forms of healthcare in the United Kingdom. Consultations include a detailed case history, manual muscle testing
When engaging in services from Innate Health you acknowledge that:
1. Innate Health does not claim to diagnose or cure medical conditions and does not replace the care of your GP or other licensed medical professionals.
2. You are encouraged to consult with your GP or relevant medical practitioner regarding any health concerns and to inform them of any nutritional or kinesiology protocol you undertake.
3. Innate Health may refer you to your GP for further investigations or tests if deemed appropriate.
4. You agree to disclose full and accurate health information, including current and past diagnoses, symptoms, medications (prescription or over-the-counter), supplements, herbal remedies, and relevant test results. You will also update Innate Health on any changes in your health status.
5. You agree to inform Innate Health if you are pregnant, planning to become pregnant, or breastfeeding, and will notify them immediately of any changes to this status.
6. All advice and interventions given are individualised based on your health presentation, case history, and responses to manual muscle testing.
7. You understand the scope and nature of nutritional therapy and applied kinesiology and will seek clarification of any recommendations or procedures you do not fully understand or feel comfortable with.
8. While every effort is made to ensure safe practice, you acknowledge that side effects or adverse reactions are possible, and no specific outcomes can be guaranteed.
9. In the event of any adverse effects, you agree to discontinue treatment, seek medical advice if appropriate and inform Innate Health immediately.
10. Your program and recommendations will be provided with a specified timeframe and review process. Continuing beyond this timeframe without review may increase the risk of adverse effects.
11. You understand that to achieve certain therapeutic effects carefully selected supplements may be recommended. These are an additional cost.
12. You understand any changes to the recommended supplement(s) brand, dose, frequency and duration increases the risk of adverse reactions and may not offer the desired therapeutic effects.
13. You understand that dietary and lifestyle changes may be needed to achieve optimal therapeutic effects.
14. All information shared with Lily Lee will be treated in accordance with General Data Protection Regulations (GDPR) and is strictly confidential unless disclosure is required by law.

Hours of Business
Normal Innate Clinic working hours are Tuesday to Friday 10.00am - 4.00pm. Exceptions to these hours will be included in Whole Practice booking system and email out of office notifications.
Clinic Locations
Innate Health clinic consultations are conducted Face to Face in Hove, and in certain circumstances by Virtual Clinic using web conferencing or Telephone.

Consultation appointment confirmations will be emailed to you and a reminder sent prior to your appointment. Please note that if you need to cancel or reschedule your consultation 48 hours notice is required to avoid incurring a cancellation fee. Please see below for our Payment and Cancellation Policy
Virtual Clinic Consultations are conducted on video via a web conferencing platform such as Google Meet. Details of how to join your consultation will be sent to you prior to your consultation. You will require access to a computer, tablet or mobile device with a WIFI connection at the time of your consultation. Consultations are deemed to start and finish at your allotted consultation times; please aim to arrive at your Virtual or Face to Face Clinic Consultation promptly. Consultations started late due to late arrival from the client will still end at the pre-agreed end of consultation time.

Clinic Response Times
We aim to respond to client questions and enquiries within two business days, and sooner where possible. Please note that our normal working hours are 10.00am -4.00pm Tuesday to Friday and that we are unable to respond on weekends or public holidays. If your query is urgent please mark it as 'urgent' within the email heading.
Payment and Cancellation Policy
Payment in full is required at the time of booking appointments. Please be courteous and provide 48 hours notice for cancellation of appointments. This helps to ensure that we can meet the needs of clients who may be waiting for an appointment. Cancellations made within 48 hours of your appointment time will be charged at the full consultation rate.
Refund Policy
Refunds are entirely at the discretion of Innate Health.
